Company Description
Our name, Monogram, symbolizes the personalized focus of our services. While the application of the latest technology and data science is critical to our success, we never lose sight that healthcare is ultimately about trusted individual relationships between healthcare professionals and their patients. Unfortunately, individuals living with advanced CKD and ESRD are negatively impacted by well entrenched systematic forces such as perverse economic incentives, poor adherence to evidence-based care, and a lack of sensitivity to the negative impacts of social determinants of health. The 21st Century Cures Act is a seminal piece of legislation designed to shift individuals living with ESRD from FFS Medicare into managed care and reverse decades and decades of sub-optimal outcomes. Monogram was born with a vision to serve health plans and other sponsors of health care and solve for these systemic problems. Our mission is to improve clinical outcomes by predicting the onset of CKD and ESRD, slowing its progression, and reducing hospitalizations, morbidity and mortality. We are founded and operated by a core group of former government programs health plan executives, including our CEO, Michael Uchrin, a world-renown nephrologist and author of over 200 published pieces of research, Dr. Raymond Hakim, and former US Senator Bill Frist M.D., a retired cardiac surgeon and recognized leader for improving the quality of care provided to human beings across the world. We welcome all stakeholders in the CKD | ESRD ecosystem to join us in our mission to measurably improve outcomes for individuals living with these conditions.

"Rapid growth"

"Fun place to work so far, hopefully more to come"
Current EmployeeHelp Desk Analyst
If you're looking to work your way up on IT then this is where you want to get your feet wet. Management and peers seem to be great as well. Family-oriented team enviornment. Take care of the team around you, then the other facilities. You will need some experience as you will be thrown into the mix, but other than that you should be fine to hit the ground running. Management is laid back for IT.

"Growing Company for Growing Individuals"
Current EmployeeCenter Director
Since working with Monogram Health, I have been warmly welcomed by the staff and Leadership Team. Monogram Health is an excellent opportunity for anyone still growing and developing in their career as well being an excellent opportunity for seasoned employees looking for a role where they can contribute to the growth of the company and its staff, as well as its patients.

"No work/life balance"
Former EmployeeRegistered Nurse Manager
The company was untruthful about the expectations of the job during the interview process, saying the travel is no longer than 90 miles from your home and there would only be “x” amount of visits in one day. Then once hired on they claim they never said any of that. Everyone was told different things about travel lengths/times and none of the management knew the actual policies. The management was very unorganized and undertrained. The field staff was also not properly trained on what the expectations were when it came to documentation. This company is new and they have a lot of kinks to work out. They are too focused on making money and not focused enough on building a strong company that will stand the test of time. Do not work here if you want a work/life balance as they want you working extra nights and weekends while salaried so they don’t have to pay overtime.
