Monogram Health supports patients suffering from Chronic Kidney Disease and End Stage Renal Disease by forming deep rooted relationships and preparing them both emotionally and physically for the challenges of managing kidney disease. Monogram Health uses next generation artificial intelligence algorithms to predict necessary and timely care to promote the delay of kidney disease progression, seamless transitions to dialysis and/or pre-emptive kidney transplant. We then utilize in-home visits to build meaningful relationships between patients and their care team that drive positive behavior change and optimize our patients’ health outcomes.

P*osition: Sr. Director, Client Performance and Operations*

The Sr. Director, Client Performance and Operations serves as the owner for one or more clients and manages all business outcomes, working in partnership with others across the organization to ensure meeting utilization goals, operational and quality metrics.

Position Requirements

  • Phoenix, AZ or Nashville, TN preferred, remote open with travel
  • Bachelor’s degree from a four-year college
  • 5-8 years’ of management experience in managed care
  • 5+ years’ experience working for value-based provider group, risk-based organization, health plan, and/or vendor contracting with health plans delegated for services
  • 5+ years’ experience as Market Director, Executive Director, Operations Director, General Manager or similar role
  • Detail oriented, excellent prioritization, and time management skills
  • Expertise partnering with internal departments to drive collaboration and results
  • Highly proficient in Microsoft Excel, Word, and Powerpoint
  • Experience working in high-growth environment preferred
  • Valid driver’s license

Roles and Responsibilities

  • Manage one or more clients as the owner for all business outcomes, working in partnership with Market and Clinical Operations to ensure sets and meets utilization goals, operational and quality metrics
  • Accountable for performance of client to reach goals on enrollment, cost savings, quality, engagement, and documentation.
  • Regular review of key operational metrics and reports to track performance
  • Proactively identify and problem solve obstacles to reaching targets
  • Cultivate strong community relations with community IPAs, PCPs, nephrologists, specialists, acute care
  • Solicit and build new potential client relationships in market- partner with business development to push forward any market growth conversations and tactics
  • Work with local team and corporate to identify and implement innovative approaches to drive net savings and improve documentation
  • Provide oversight for continuous quality improvement initiatives. Build and maintain positive relationships with health plan business and clinical counterparts
  • Serve as the key account manager for the health plan relationship, including but not limited to leading Joint Operating Committee meetings, attending key executive meetings and serving as a partner for operational needs as they arise.
  • Collaborate with the clinical and operational leadership to plan and execute initiatives to continuously improve the clinical delivery model
  • Monitor all metrics to identify opportunities for process improvements and/or staff education to produce optimal outcomes
  • Participate with senior management on special projects/initiatives
  • Other duties as assigned

If you're looking to work your way up on IT then this is where you want to get your feet wet. Management and peers seem to be great as well. Family-oriented team enviornment. Take care of the team around you, then the other facilities. You will need some experience as you will be thrown into the mix, but other than that you should be fine to hit the ground running. Management is laid back for IT.

The company was untruthful about the expectations of the job during the interview process, saying the travel is no longer than 90 miles from your home and there would only be “x” amount of visits in one day. Then once hired on they claim they never said any of that. Everyone was told different things about travel lengths/times and none of the management knew the actual policies. The management was very unorganized and undertrained. The field staff was also not properly trained on what the expectations were when it came to documentation. This company is new and they have a lot of kinks to work out. They are too focused on making money and not focused enough on building a strong company that will stand the test of time. Do not work here if you want a work/life balance as they want you working extra nights and weekends while salaried so they don’t have to pay overtime.
